Living with type 1 diabetes can be challenging. Blue Circle Health provides free, personalized care, education and support to adults living with T1D to help you get back in the driver’s seat – at zero cost to you. We act as an extension of your care team as the extra support you deserve.

Are You Eligible?

To sign up for Blue Circle Health, you must meet the following requirements:

Be diagnosed with Type 1 Diabetes
Be 18 years or older
Live in Florida, Maine, Vermont, Ohio, or Delaware
Speak English and/or Spanish
Have access to the internet and a smart phone or computer to do video calls with the care team
Have a primary care provider or are willing to be connected to one
Are not currently pregnant
